New Drug-Eluting Cardiac Stents and Dual Antiplatelet Therapy: How Short is Too Short?

Abstract
The 2009 APSF Newsletter highlighted concerns regarding thrombosis after drug-eluting stent placement amidst evolving dual anti-platelet therapy (DAPT) recommendations that are still debated today. Since then, several studies have demonstrated the advantages of newer-generation drug-eluting stents, suggesting the feasibility of shorter DAPT durations while maintaining safety, particularly in patients at high risk of bleeding. Societal guidelines recommend tailored DAPT strategies that consider individual risk profiles and balance ischemic protection with bleeding risks. These guidelines stress the importance of multidisciplinary decision-making and strategic facility resource planning when managing patients requiring noncardiac surgery following stent placement.
